I understand that the Michigan Dolphins are a traveling team. I understand the team travels throughout Oakland and Wayne County for regular season games. I understand that there is the possibility that my child(ren) would have to travel to Kentucky and Florida if the team advances. I understand there also may be out of state tournaments that the team will compete in.

I understand once equipment and uniforms are passed out to my youth, it is up to myself the parent/guardian and the youth to take care of the items. I understand that ALL equipment must be returned at the end of the season. Type full name in the box below

Youth State ID-This can be obtained at the Secretary of State (you/your youth's original birth certificate/SSN card/report card/piece of mail will need to be taken to your appointment with them).

2025 Date Physical Form-this can be performed by your youth's pediatrician/doctor or by a clinic.
