I understand that the Michigan Dolphins are a traveling team. I understand the team travels throughout Oakland and Wayne County for regular season games. I understand that there is the possibility that my child(ren) would have to travel to Florida if the team advances. I understand there also may be out of pre-season and off season state tournaments that the team will compete in.

Youth State ID-This can be obtained at the Secretary of State (you/your youth's original birth certificate/SSN card/report card/piece of mail will need to be taken to your appointment with them).

2026 Date Physical Form-this can be performed by your youth's pediatrician/doctor or by a clinic.
